A very unprofessional mistake at that level. Hmmm, do you blame the coach for the lack of coached drilling or the swimmer[s]?
US Track and Field has a long history of this sort of nonsense.

The Americans touched first in three minutes 30.06 seconds before the stadium announcer said they had been disqualified "for an early exchange". France clocked a time of 3:31.51 to take gold, with Australia winning silver in 3:31.64 and Japan bronze in 3:32.26.
The Americans were denied what would have been a 12th triumph in the event in 15 championships since the inaugural edition in Belgrade in 1973.

Its not the coaches fault in swimming. The swimmers have been doing relays for years. They know that they cannot jump early.
I realize we are in a world of celebrity (kind of) coaches who get credit and blame for performances but its the swimmers who should get the credit or the blame.
I remember when we were at SCAQ LCM regionals at Conejo Valley and Dan Leonard said, "See this SCAQ logo on our caps. It's a target for DQs (We had a DQs including Clay but we still won the meet.), he went on to say, when your relay swimmer swims in, shake their hand then ask the official for permission before you dive in.
Was funny.
